According to the latest UNICEF report published on January 12, 2025, the situation has worsened considerably compared to the previous year, with a rapid and widespread spread of the disease. As of January 5, 2025, UNICEF specifies in its report, 46 of Burundi's 49 health districts have reported cases of Mpox, compared to only 33 the previous year. Out of 3,355 people screened, 1,767 tested positive, revealing the scale of the epidemic. Children are particularly vulnerable, representing 37% of positive cases among those under 15, making them the most affected age group. Bujumbura province remains the epicenter of the epidemic, with its health districts being the hardest hit. In the same report, UNICEF stresses that it is committed to continuing its support to the Burundian government, particularly by providing the kits needed to fight the epidemic. - 13 January 2025 ... Burundi Mpox Level 3 Emergency HIGHLIGHTS As of January 05, 2025, the epidemic has spread from 46 health districts out of 49, or 93.8% of the districts. Of these, 17 are currently active, having reported at least one confirmed case in the last 21 days compared to 26 in November and 33 in October. A total of 1,767 cases out of 3,035 confirmed cases reported were recorded in three health districts of Bujumbura (Bujumbura North, Center and South), or 58,2%. Bujumbura North continues to be the most affected district with 1,142 cases, or 37.6% compared to 38.1% in November. Children under 15 years of age remain the most affected group with 37.8% compared to 39.5% in November; children under 5 years of age represent 20.6% of total confirmed cases, compared to 21.7% in November. However, young adults between 20 and 30 years of age now represent 30,7% of cases, compared to 29.7% in November. 6,924 people affected by Mpox, including 3,557 children (51,3%), have benefited from psychosocial support and mental health services in the treatment centers and in the community. It is a two-month-old infant, who lives with his mother in the Luvangira displaced persons site. According to medical sources, the case was suspected on January 5, when samples were taken for analysis. These confirmed that the child had monkeypox. Two other suspected cases were also reported in the same displaced persons site. Archipe Kule Kyusa, the supervising nurse in charge of preventive activities and epidemiological surveillance, indicates that all measures have been taken to follow up on contacts at risk and thus avoid the spread of the disease. "The health zone has just reported a positive case of Monkey pox. It is a two-month-old child and curiously his family lives in a displaced persons site.Our fear is that the case is where people are crowded together, there is a risk that the spread of the disease will be great ," fears the nurse. This is why the health zone and the ECZ team thought it better to list the contacts. In the meantime, there were two other people who presented the same signs and are already at the Monkey Pox Treatment Center (CTM) in Oicha, he adds. More than 9,500 cases in the DRC including 43 deaths Since its first detection in September 2023, clade Ib has been detected in eight provinces of the DRC: South Kivu, North Kivu, Kinshasa, Kasai, Tshopo, Tanganyika, Haut-Katanga and Mai Ndonbe. In Africa, from January 2024 to January 5, 2025, 14,700 confirmed cases of Mpox, including 66 deaths, were reported by 20 countries. The most affected country remains the Democratic Republic of Congo (9,513 cases including 43 deaths). This is followed by Burundi (3,035 confirmed cases, including one death) and Uganda (1,552 confirmed cases, including 12 deaths). Burundi and Uganda continue to report between 100 and 200 new smallpox cases per week, with a tendency to plateau, possibly due to a drop in surveillance and reporting during the December holiday period. Community transmission has also been reported in other countries on the continent, such as Kenya and Rwanda. Travel-related cases have been reported in Zambia and Zimbabwe. In addition, although no cases have been confirmed to date in Tanzania, the identification of travel-related smallpox cases in Tanzania suggests undetected transmission in that country. Mpox cases in Europe and North America linked to travel to Africa Outside Africa, eleven countries have detected MPXV clade Ib. Sweden, Thailand, the United States of America and Canada have detected one case each among travellers from affected countries in East and Central Africa. For WHO, these recent travel-associated smallpox cases highlight the risk of undetected transmission within countries, underscoring the need for enhanced surveillance and timely reporting to improve global monitoring and prevent international spread. A travel-related case of Mpox due to clade Ib in Germany (the second importation) led to secondary transmission at home. Germany has thus confirmed six cases: three cases among travellers from affected countries in East Africa and three household contacts of one of these travellers. MPXV clade Ib has been detected in three other countries, Belgium, China and France, directly or indirectly linked to travel to the affected countries in Central Africa. Kosovo reported its first case of MPXV. No information on the MPXV clade is yet available. Less than a week ago the UK HSA announced the 6th clade Ib Mpox case in the UK (3 imported, 3 locally acquired). Today they are announcing their 7th case; detected in an individual who recently travelled from Uganda. So far, we've seen 11 non-African nations report imported cases of this Mpox Ib clade; Sweden, Thailand, India, Germany, the United Kingdom of Great Britain and Northern Ireland, the United States of America, Canada, Pakistan, Belgium, China, and France. Several countries, including China, Belgium, Germany, France and the UK have reported limited secondary transmission. The latest update from the UKHSA is unusually brief. Clade II, which was found primarily in the DRC, was the more severe of the two. But for years researchers have warned that the Monkeypox virus was becoming more transmissible, and it had the ability to mutate into a more dangerous variant. A 2016 study (see EID Journal: Extended H-2-H Transmission during a Monkeypox Outbreak) looked at a large 2013 outbreak of Monkeypox in the DRC and suggested that the virus's epidemiological characteristics may be changing (possibly due to the waning smallpox vaccine derived immunity in the community). The DRC had reported a 600% increase in cases over both 2011, and 2012. The authors also cite a higher attack rate, longer chains of infection, and more pronounced community spread than have earlier reports. Like all viruses, Monkeypox continues to evolve and diversify, as discussed in the 2014 EID Journal article Genomic Variability of Monkeypox Virus among Humans, Democratic Republic of the Congo, where the authors cautioned: Small genetic changes could favor adaptation to a human host, and this potential is greatest for pathogens with moderate transmission rates (such as MPXV) (40). The ability to spread rapidly and efficiently from human to human could enhance spread by travelers to new regions. And in the spring of 2022, a new clade IIb began spreading internationally, forcing the WHO to declare a PHEIC (Public Health Emergency of International Concern). While that outbreak slowed over time, at least 100,000 cases were reported outside of Africa. In mid-August of 2024 the WHO declared the Mpox outbreak in Central Africa to constitute a PHEIC after reports that clade (Ib) of the virus had begun to spread outside of the DRC (see More African Nations Reporting Mpox - Africa CDC Mpox Update (Jul 30th)). Today, there are 4 officially recognized clades of the Mpox virus (Ia, Ib, II, IIb), and more could emerge over time. A reminder of what can happen when an emerging virus is allowed to simmer for years without serious interventions. Last month the WHO published a new, updated risk assessment, which puts the risk of national and international spread of clade Ib as HIGH and Ia, II, and IIb as MODERATE. So far, at least 11 non-African nations have reported imported clade IIb cases over the past 6 months. As our collective immunity from the smallpox vaccine - which was discontinued in the late 1970s - wanes, there will be increased opportunities for emerging pox viruses to make a comeback. Mpox in the DRC: residents of a Kinshasa shanty town on the front line Published: February 4, 2025 7:29am EST Author Yap Boom Professor in the faculty of Medicine, Mbarara University of Science and Technology - MPOX is historically a rural disease in the DRC. This microcosm of Kinshasa highlights the complex challenges of managing the epidemic in a city. Fighting on two fronts With a population of over 17 million, Kinshasa is Africa's largest megacity . Pakadjuma is one of the city's many overcrowded neighborhoods where people live in extreme poverty. Kinshasa, often called “Kin the Beautiful,” is facing a unique crisis in the fight against MPOX. Both strains of the virus, clade Ia and clade Ib, are circulating simultaneously in the city. This is the first time this has happened. Clade Ia , which is transmitted primarily from animals to humans and then within households through touch, has been endemic in Africa for decades. Clade Ib is a new strain that is primarily contracted through sexual contact. This is the strain that has spread rapidly across 21 African countries during the current outbreak in East and Central Africa. This dual transmission makes the fight against MPOX even more complicated: how do we tackle a public health crisis rooted both in intimate human relationships and in structural inequalities such as living in crowded areas? Although the strains are treated clinically similarly, their spread and transmission differ. Clade Ia is primarily associated with zoonotic (animal-to-human) transmission in rural areas. Animal surveillance and community education are needed to control spillovers. Clade Ib, which has higher human-to-human transmissibility, requires intensified contact tracing, vaccination and preventive measures in urban and peri-urban areas. Adapting strategies to these differences is essential to contain the epidemic. - The fight on all fronts The fight against the MPOX epidemic in Kinshasa requires a multifaceted approach: Vaccination: Widespread vaccination campaigns offer the best hope for controlling the outbreak in hotspots such as Pakadjuma, where contact tracing is nearly impossible. In these cases, the entire community must be vaccinated. DJ...Very likely we may see new clades besides the now 4 known clades (l, lb, ll, llb). Mpox is also on its way to become a pandemic...for now flu+CoViD are my top of the list...but measles, TB, Mpox (etc) are ALL becoming major problems... Human immunity-as a species-may be damaged...(and also lots of mammals immunity in bad shape...).
